Time: 6:00 - 10:00 p.m. Dinner served at 7:00 p.m.
Location: The Willow Creek Country Club, 8505 Willow Creek Drive, Sandy, Utah 84093
Cost: $40 per person
Dinner Buffet: Soon to be announced.
Beverages: Cash bar, beverage table with soft drinks, iced tea, lemonade and coffee

Unfortunately, NO registrations accepted after November 25 or on the night of the event so please register early.
